The various forms from this packet must be completed and submitted by Feb 15.

If the "Email" column contains a "Yes", then the form may be completed electronically and e-mailed as an attachment to the Central Office.

To open PDF files, the free Adobe Acrobat Reader is required. Files cannot be saved with Adobe Reader. Any word processing software should be able to open and save RTF files. Microsoft Excel files can be opened with the free Excel Viewer, but cannot be saved with it. The free Open Office software can open and save Excel files.
